Release and Hold Harmless Agreement

1. I, the undersigned participant (or parent/guardian if the participant is under 18 years of age), hereby acknowledge that participation in activities organized by North Atlanta Tennis Academy, LLC (NATA) involves inherent risks, including but not limited to physical injury, property damage, or other losses.

2. I voluntarily agree to assume all risks associated with my (or my child's) participation in NATA activities, whether known or unknown, foreseeable or unforeseeable.

3. I hereby release, waive, discharge, and covenant not to sue North Atlanta Tennis Academy, LLC, its owners, directors, officers, employees, volunteers, agents, and all other persons or entities acting on its behalf (collectively referred to as "Releasees"), from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury that may be sustained by me (or my child), or to any property belonging to me (or my child), while participating in NATA activities.

4. I agree to indemnify and hold harmless the Releasees from any loss, liability, damage, or costs they may incur due to my (or my child's) participation in NATA activities.

5. I understand that this Release and Hold Harmless Agreement is intended to be as broad and inclusive as permitted by the laws of the State of Georgia and agree that if any portion is held invalid, the remainder of the agreement will continue in full legal force and effect.

6. I affirm that I am of legal age and am freely signing this agreement, or that I am the parent or legal guardian of the participant and am freely signing this agreement on their behalf.

7. I have read this Release and Hold Harmless Agreement, understand its terms, and agree to be bound by it.

Medical Release Form

I, the undersigned parent/guardian of the participant registered for the North Atlanta Tennis Academy, LLC (NATA) summer camp, hereby authorize the staff of NATA to act for me according to their best judgment in any emergency requiring medical attention. I hereby waive and release NATA, its staff, and representatives from any and all liability for any injuries or illnesses incurred while participating in the summer camp program.

I understand that:

1. I am responsible for informing NATA of any medical conditions, allergies, or medications that may affect my child's participation in the summer camp.

2. In case of an emergency, NATA staff will attempt to contact me or the emergency contact provided during registration.

3. If I or the emergency contact cannot be reached, I authorize NATA staff to secure appropriate medical treatment for my child.

4. I am responsible for all costs of medical attention and treatment provided to my child.

5. NATA does not provide medical insurance for participants.

6. I give permission for NATA staff to administer basic first aid and over-the-counter medications (e.g., pain relievers, allergy medication) if deemed necessary.

7. In the event of a medical emergency, I authorize NATA staff to release any medical information to the treating medical personnel.

8. This release is valid for all NATA summer camp activities during the current season.

PHOTO AND MEDIA RELEASE FORM

North Atlanta Tennis Academy, LLC (NATA) requests your permission to use photographs, video footage, and/or audio recordings of you (or your child) for promotional and educational purposes. Please read the following and indicate your choice by selecting "Yes" or "No":

I hereby grant permission to North Atlanta Tennis Academy, LLC, its employees, and authorized representatives to take photographs, video footage, and/or audio recordings of me (or my child) during NATA activities and events. I further grant NATA the right to use, publish, and reproduce such photographs, video footage, and/or audio recordings for any lawful purpose, including but not limited to:

- Websites and social media platforms
- Printed materials such as brochures, newsletters, and advertisements
- Educational and training materials
- Other promotional and marketing activities

I understand that no compensation will be given for the use of these materials.

Do you grant permission for North Atlanta Tennis Academy, LLC to use photographs, video footage, and/or audio recordings as described above?

Payment/Refund/Rain Policy

1. Directors of the program will assign groups based on skills, maturity, and development as they see fit for the best learning environment.

2. Payments must be complete for programming before the camp week starts.

3. Players must be able to follow instructions, especially when it comes to the safety of the players and those around them.

4. Simply leaving the court without permission on a regular basis can result in being removed from the camp. Instructors must know when players are going to leave the court for safety reasons.

5. Rainouts happen: Rain makeups are done by choosing a day in any of the following camp weeks. The rain makeup must be done during the camp season and will expire when camp season is over.
